Welcome to our Terms of Use page of the My Health Book application. These terms of use will guide how you use and interact with our application and its related services. By using our services, you agree to abide by these terms and conditions. In case you do not agree, please do not use our services.

Service Description

The application is designed to help users maintain their medical records in digital form in a systematic manner. The user can store, access, and manage the medical information. The application uses OpenAI services to summarize the given reports only when the user enables “Wanted AI Findings”.

Eligibility

To use the service, you must

  • Provide the correct information to create an account.
  • Upload & submit the reports related to medical purposes.
  • Hold a minimum of 15 years of age.

User Account

To use our service, users need to have and maintain a user account, and for this purpose:

  • The user needs to provide accurate information when creating an account.
  • Maintain the confidentiality of the login credentials.
  • The user will be responsible for the activities under their account.
  • It is the user’s responsibility to notify us of any unauthorized activity.

Acceptable Use

Users are subject to certain restrictions when using our services. They must use the application solely for its intended purpose: storing and retrieving medical reports. They must understand that no offensive or harmful submission is acceptable. Also, no illegal activities will be bearable.

AI-Generated Content Disclaimer

The application uses OpenAI services to generate summaries of medical reports when the AI summary feature is used. Therefore, you understand and agree that there may be mistakes as there is no 100% accuracy guarantee.

Intellectual Property

The design, content, text, logo, and features of the application belong to Moon and are protected by intellectual property and copyright laws. Therefore, users cannot reproduce, distribute, or commercially exploit the platform without permission.

User Ownership

Users own the data they provide during account creation. Additionally, they retain ownership of all data they submit to the application, including medical reports, which are stored. The data is stored securely, and the application complies with HIPAA standards.

Availability of Service

Our application is available 24/7, but we cannot guarantee uninterrupted service. We reserve the right to change features, update the version, or temporarily suspend the service for maintenance.

Limited Liability

We are not liable for any indirect or consequential damages, including loss of data, incorrect summary results, or service interruption. So, always use the service at your own risk.

Termination of the Service

We reserve the right to terminate a user’s service by suspending the account. The action is taken when the terms are violated, the platform is misused, or any other illegal or suspicious activity is found.
